School Program Referrals

Please complete ONLY as a TLDSB/PVNCCDSB referral, on behalf of a student. Please ensure parent/guardian consent prior to completing as we will need to contact parent/guardian throughout program. This form is designed for In-School Mentoring (ISM) programming. An ISM match occurs from October until June, unless otherwise stated. A student can become enrolled in ISM at any point throughout the school year. ISM matches to meet 1 hour/week either in-person during school hours or virtually through a secure Google Meets link set up by a school liaison.     In-School Mentoring (ISM) is either an in-person program, or virtual depending on the needs identified by the school/mentor. The student will meet with their mentor always during school hours.
